Tips to Prepare for Your Appointment

Before You Visit

Before you come for an appointment, it’s a good idea to do the following things:

If your pet has been seen by another veterinarian, obtain a copy of your pet’s medical records. The easiest way to do this is to call your previous veterinary hospital and ask to have your pet’s medical records faxed to us. Please try to do this several days prior to your appointment. Our fax number is 212-501-9614.

Make notes concerning the environment, diet, and general condition of your pet. We will ask you to fill out a detailed history form! The patient history forms are available online for you to review, or to print and fill out prior to your visit.

Contact us to find out if you should bring a fresh stool or dropping sample from your pet. We will generally recommend a fecal exam for any pet that has never been checked for intestinal parasites or is having abnormal stool. If a fresh sample cannot be collected within an hour of your appointment, you can place a sample in a sealed plastic container and refrigerate it for up to 24 hours.

During your visit, our doctors will examine your pet and make sound recommendations for their health. You will always be given a written treatment plan and cost estimate for our recommendations. It is our way to ensure that you are aware and agree to the care we want to provide for your pet.
